I read this around the early 2000s (well, I listened to the audiobook of it) when I was young--so I'm assuming the genre was YA.
It was definitely fantasy. From what I can remember:
- there was a man in our earth/our reality who (I wanna say) his wife died or was dying.
- he finds a way to go into another world
- i believe there were talking animals of some sort
- the main character had an issue with those in power & wanted to challenge them for some reason
I feel like, at the end of the first book, he went back to earth--something with him driving in his car.
I'm desperate!

Haven't read much YA but from the description, Lord Foul's Bane comes to mind (not YA), though I think it would be fairly distinctive. The protagonist is a leper and becomes outcast, is hit by a car and wakes up in a fantasy world where he's believed to be a reincarnation of a particular hero name Berek Halfhand. I didn't read it beyond the intro, but I remember the hero's name because of how similar it was to Tolkein's Beren One-hand. It's the first in a series.
